Getting your child to use an inhaler effectively is harder than it sounds. Whether your child has asthma, reactive airway disease, or another respiratory condition, the device you choose—and how you use it—directly affects whether medication actually reaches their lungs.

What's the Difference Between a Nebulizer and a Spacer?

A nebulizer is a powered machine that converts liquid medication into a fine mist you inhale through a mask or mouthpiece. Your child sits still while breathing normally—no coordination required. The process typically takes 5 to 15 minutes.
A spacer (also called a holding chamber) is a tube you attach to a standard metered-dose inhaler (MDI). When your child presses the inhaler, medication enters the spacer where it slows down. They then inhale from the spacer at their own pace. This takes seconds, not minutes.

The downsides are real, though. Nebulizers are bulkier, louder, slower, and more expensive. They're less portable. Many children find the mask uncomfortable. And the visible mist can feel intimidating to a young kid.
Spacers: The Practical Workhorse
Spacers have become the standard recommendation for most children because they're simpler and faster.
Ease of use: You attach it to the inhaler, press once, and your child breathes normally. No mask anxiety. No waiting for treatment.
Portability: A spacer fits in a bag. You can use it at school, grandma's house, or on a plane. A nebulizer requires power and takes up real space.
Speed: Spacer treatments take 10 to 30 seconds. Nebulizer treatments take 10 times longer.
Cost: Spacers cost far less upfront and have no ongoing power costs.
That said, spacers require slightly more active participation. Your child needs to put the mouthpiece in their mouth (or keep the mask on their face). For very young toddlers or extremely resistant kids, this alone can be a deal-breaker.

If they're playing with a toy, that's fine—they're still breathing. Treatment ends when the cup is empty or mist stops appearing.
With a spacer: Attach it firmly to the inhaler. Press the inhaler once into the spacer. Your child then takes 5 to 10 slow, deep breaths through the mouthpiece (or continues normal breathing if using a mask). The goal is to get the medication into the spacer first, then into their lungs—not to inhale during the press.
A common mistake: pressing the inhaler multiple times quickly. Press once, then let your child breathe from the spacer. Pressing more than once wastes medication.
Another mistake: rinsing or wiping down spacers without letting them fully air dry. Residual moisture can affect how the one-way valve functions.
Cleaning and Maintenance
Spacers accumulate a static charge that can make medication particles stick to the inside walls instead of reaching the lungs. Many parents don't realize this.

Making Treatment Work at Home
Getting kids to cooperate with medication is half the battle.
Nebulizers work best when you're not forcing compliance. A mask that's too tight or a machine that sounds scary will create resistance. Let your child hold the mask if they're old enough, or decorate it with stickers. Some families use nebulizer time as TV or audiobook time.
With spacers, don't wrestle. If your child refuses the mouthpiece, try a mask instead. If they hate the whole thing, take a breath and try again in 10 minutes. A brief, calm treatment beats a stressful battle that accomplishes nothing.
Both devices work best when your child isn't already upset. Stress tightens airways and makes breathing harder.
What Actually Matters When Choosing
Your child's age, the prescribed medication, and your family's lifestyle all factor in. A 2-year-old during an acute flare probably needs a nebulizer. A 6-year-old with daily maintenance medication probably needs a spacer they can grab quickly.
Ask your child's doctor which device they recommend for your specific situation. Ask for a demonstration of proper technique. Ask how often to clean the device. Get answers to these practical questions—they matter more than any theoretical advantage.

Neither device is inherently superior. The best one is the one your child will actually use correctly. A spacer your teenager refuses is worthless. A nebulizer that's so complicated you skip treatments won't help.
Start with what your doctor recommends. If it's not working after a genuine trial period, speak up. Switching devices is a normal part of managing childhood respiratory conditions. Your job isn't to stick with the "right" tool—it's to get the medication into your child's lungs, consistently and calmly.
